Step1 - Complete Activation Form
This form is used to provide contact information for a venue that is being activated within the CityCheers and ExpressCheck network. It also is used to provide the name and contact information for the owner, general manager, trainer, and/or person with authorization for the venue to fill out and complete this form. The "trainer" is a member of the venue staff that will have the primary responsibility for training and promoting CityCheers with the rest of the staff. The referral section will be populated if you were referred here by somebody. You do not need to fill in the referral section yourself.
